I understand that if my team is not accepted, my entry fee will be returned in full. I understand and agree that once my team is accepted and later wiithdraws, the entry fee is forfeited. No refund will be made in the event of cancellation or the shortening of the tournament due to inclement weather.

Please read the registration information provided, especially the eligibility requirements and credentials required.

Eligibility: Each player must have his/her birth certificate, if under 16, or his or her driver’s license or birth certificate, if 16 or older. Other credentials needed at check-in are roster and waiver form.

Tournament Dates: Saturday, 7/21/12, beginning at 8:00 AM; Sunday, 7/22/12, beginning at 8:00 AM.

Tournament Location: Rotary Soccer Park Smyrna, TN

Tournament Fee: $120.00 per team. Please remit payment with this application by check to the Stones River Futbol Club by July 1st, 2012.
